Job summary

Cenikor Foundation in Deer Park, TX is seeking a clinical team member to deliver relapse prevention education and life skills groups, to document promptly, and to coordinate care with staff and community resources. This role emphasizes compassionate client support, documentation accuracy, and HIPAA compliance.

The ideal candidate will have experience in substance use counseling, strong communication, and the ability to work within a multidisciplinary team to ensure safe, effective discharge

Qualifications

• Must have good oral and written communication skills.
• Experience in education and therapeutic counseling for individuals and groups.
• Ability to identify appropriate resources for client development and provide substance use counseling.
• Capable of discerning facts from opinions and remaining objective when listening to conflicting opinions.
• Experienced with diverse populations.
• Strong time management to meet deadlines.
• Knowledge of MS Outlook calendar expected.
• Uphold Cenikor's core principles in daily work.

Responsibilities

• Conduct relapse prevention education and life skills group sessions. Document and upload notes within 24 hours of group completion.
• Facilitate groups as needed and in collaboration with facility staff.
• Maintain a safe environment for client recovery in coordination with all staff.
• Communicate with individual, family supports and wellness team to reduce fragmentation in services at discharge.
• Educate clients and families about discharge options and community resources.
• Participate in weekly clinical staff meetings and case management sessions.
• Ensure HIPAA compliance and client confidentiality across all records.
• Maintain updated database of referral resources within the community and state.
• Review deliverables and report on SOW progression; monitor compliance.
• Complete required trainings including state certifications as applicable.
