Avenues Recovery Center in Baton Rouge, LA is seeking a dedicated Case Manager to advocate for clients and coordinate barriers to treatment. You will initiate intake case management, monitor insurance, and arrange housing, financial aid, and post-discharge planning to support successful recovery.

Ideal candidates have a High School diploma or GED and at least 1 year in the treatment field, with strong communication, multitasking, and problem-solving skills and a compassionate, team-oriented

Qualifications

• High School diploma or GED preferred.
• Min. 1 year of experience in the treatment industry preferred.
• Must possess strong communication and interpersonal skills with a compassionate approach.

Responsibilities

• Advocate for client, addressing and resolving barriers to treatment.
• Initiate case management for each client upon intake, addressing pressing needs.
• Oversee client accounts, ensuring insurance is active throughout duration of treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange housing, financial aid, legal aid, vocational training, etc. as necessary for post-discharge.
• Facilitate comprehensive discharge planning (continuing care, housing, sober support).
