Highland Rivers Behavioral Health in Georgia is seeking a Case Manager for the ROC Clubhouse program to support youth with mental health needs. The role involves building self-esteem, resiliency, and life skills within a structured, supportive environment.

The position operates Mondays through Thursdays 10:30am–7:30pm and Fridays 8:00am–4:00pm, offering professional growth in a community health setting. Strong collaboration with service providers is essential.

Qualifications

• Bachelor’s degree required in Psychology, Social Work, Sociology, Human Services, Criminology, or related field.
• Ability to work independently and manage time effectively.
• Computer skills to learn and use electronic medical records.
• 1–2 years of behavioral health or substance use disorder experience preferred.
• Criminal background check and driving record requirements in place.

Responsibilities

• Assist individuals with service needs and coordinate rehabilitation services per recovery plan.
• Partner with individuals and providers to develop the individual service plan.
• Provide assistance/support in crisis situations.
• Conduct assessments, monitoring, and assistance to individuals.
• Maintain professional relationships with individuals, families, and coworkers.
• Complete documentation and ensure eligibility criteria are met or referred to appropriate providers.
• Manage caseload with authorized services, up-to-date charts, and closed records per guidelines.
• Perform other duties as assigned.

JOB SUMMARY: The ROC ("Reaching Our Community") Resiliency Support Clubhouse help youth with mental health issues build self-esteem, resiliency and life skills in a fun and safe environment. The Case Manager will work from Mondays through Thursdays from 10:30am to 7:30pm and Fridays from 8:00am to 4:00pm.

Duties and Responsibilities include:

• Assist individuals with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the individual’s individual recovery plan.
• Partner with the individual and service providers in the development of the individual service plan.
• Provides assistance and support in crisis situations.
• Provides assessment, monitoring, and assistance to individuals.
• Establishes professional and ethical relationships with individuals, families, and coworkers
• Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community.
• Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to Agency guidelines.
• Other duties as assigned.

Minimum Qualifications

• Bachelor’s degree in Psychology, Social Work, Sociology, Human Services, Criminology, or a related field of study.
• Skilled in working independently and implementing time management strategies.
• Computer skills sufficient to learn and utilize the electronic medical records.
• 1–2 years of professional experience in behavioral health or substance use disorder services is preferred.
• Criminal background check clearance, official academic transcripts, valid driver’s license/ID, drug screen, TB test, and an eligible 7-year Motor Vehicle Report.
• Bilingual abilities a plus.
