The Association for Individual Development (AID) is a non‑profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Qualified Mental Health Professional (QMHP) for the Community Support Team who demonstrates this mission and wants to work for an organization that makes a difference in the community.

Responsibilities
  • Attend scheduled work hours and be flexible to meet client and program needs as assigned by Program Manager or Director.
  • Respond to calls from shift start to shift end and upon being dispatched.
  • Act as primary triage/delegate and absorb first calls.
  • Provide independent program coverage when scheduled.
  • Provide effective individual crisis counseling, including telephone counseling services.
  • Consult with family members, case managers, and other personnel as needed.
  • Use intervention tactics to reduce symptomology.
  • Complete and sign required documentation on a timely basis, including CST billing within the next two shifts and prior to program billing deadlines.
  • Log to supervisors at the end of each shift.
  • Attend clinical supervision at least once per month and as assigned.
  • Attend and positively contribute to mandatory weekly CST meetings.
  • Acquire and maintain certifications: First Aid, Adult CPR, Non‑violent Crisis Intervention (CPI), Medication Training, IM+CANS.
  • Provide direct services to clients at least 70% of work hours, 70% off‑site.
  • Ensure care integrates all facets of clients’ lives (mental/emotional health, physical health, employment, education, social/family relationships, substance use, housing).
    • Maintain records according to Rule 132, payers, and accreditation bodies.
    • Complete clinical assessments: Intake, IMCANS, LOCUS, HRA, consents, billing notes.
    • Develop and implement crisis and treatment plans in collaboration with client and team.
    • Participate in treatment planning meetings.
    • Review and revise Individual Plan (IP) and complete corrections in the allotted timeframe.
  • Maintain open and effective communication with agency, providers, family, and others.
  • Document medication distribution accurately in cx360.
  • Recognize emergency situations and take appropriate action per procedure.
  • Promote awareness of mental health internally and externally.
  • Fulfill other functions on the team as needed.
  • Promote awareness and respect for the diversity of clients, employees, families, and stakeholders.

Qualifications
  • Education: Master’s degree in a human services field required.
  • License/Certification: ASSIST and SAMHSA SOAR certification preferred.
  • Experience: At least one year working with individuals with mental illness; knowledge of DSM‑V‑TR and Rule 132; behavioral health experience preferred.
  • Physical: Ability to provide services in clients’ homes and community; navigate stairs regularly.
  • Equipment: Proficiency with Microsoft Windows, Applications, copy machine, fax machine, telephone with voicemail system.
  • Additional Requirements: Flexibility to work evening/overnight/weekend/rotating holidays; on‑call shift availability; cover non‑scheduled shifts; transport clients; maintain automobile liability insurance; valid Illinois driver’s license with good record; comply with certification requirements (First Aid, CPR, Nonviolent Crisis Intervention, CEU’s and others).
