Southcoast Behavioral Health, located in Dartmouth, Massachusetts, is a premier mental health treatment center with 192 beds. We serve pediatric, adolescent, adult, and geriatric patients, as well as adults with co-occurring mental health and substance use disorders.

PURPOSE STATEMENT:

Manage the client's/patient's/resident’s cases with a solid background of clinical knowledge and expertise, and may be responsible for providing some therapeutic services to clients/patients/residents.

ESSENTIAL FUNCTIONS:
  1. Provide case management and care coordination services for clients/patients/residents in the facility.
  2. Coordinate continuum of care activities for assigned clients, ensuring optimal utilization of resources, service delivery, and compliance with external agencies and referral sources.
  3. Conduct psychosocial assessments and facilitate individual, family, and group sessions.
  4. Coordinate treatment and discharge activities as needed.
  5. Develop, implement, manage, and evaluate client/patient care plans to ensure quality care.
  6. Collect, correlate, and provide clinical data to the treatment team.
  7. Convey medical criteria and clinical information between the insurance provider and treatment team as needed.
  8. Correlate clinical data for the business office as required.
  9. Coordinate issues or activities relevant to the treatment team in a timely manner.
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
  • Bachelor’s degree in Social Work, Counseling, or a related human service field required.
  • One or more years of experience working in a behavioral health setting preferred.
  • Experience with the patient population served by the facility preferred.
  • Experience in case management and/or utilization review preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
  • Current license required within the state of operation, if applicable.
  • CPR and de-escalation/restraint certification required (training provided upon hire).
  • First aid certification may be required based on state or facility policies.
