Southcoast Behavioral Health, located in Dartmouth, Massachusetts, is a premier mental health treatment center with 192 beds. We serve pediatric, adolescent, adult, and geriatric patients, as well as adults with co-occurring mental health and substance use disorders.

Purpose Statement

Manage the client's/patient's/resident’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic services to clients/patients/residents.

Responsibilities
ESSENTIAL FUNCTIONS:
  • Provide case management and care coordination services for clients/patients/residents in the facility.
  • Coordinate continuum of care activities for assigned clients, ensuring optimal resource utilization, service delivery, and compliance with external agencies and referral sources.
  • Conduct psychosocial assessments and facilitate individual, family, and group sessions.
  • Coordinate treatment and discharge activities as needed.
  • Develop, implement, manage, and evaluate client care plans to ensure quality care.
  • Collect and provide clinical data to the treatment team.
  • Convey medical criteria and clinical information between insurance providers and the treatment team.
  • Correlate clinical data for the business office as required.
  • Coordinate issues or activities relevant to the treatment team in a timely manner.
Qualifications
EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
  • Bachelor’s degree in Social Work, Counseling, or related human service field required.
  • One or more years’ experience in a behavioral health setting preferred.
  • Experience with the patient population served by the facility is preferred.
  • Experience in case management and/or utilization review is preferred.
Licenses/Designations/Certifications
  • Current license required within the state of operation, if applicable.
  • CPR and de-escalation/restraint certification required (training provided upon hire).
  • First aid certification may be required based on state or facility policies.
