Job summary

Lifepoint Rehabilitation's Baptist Memorial Rehabilitation Hospital in Germantown, TN is recruiting a PRN Case Manager. This role coordinates interdisciplinary care plans, discharge planning, and patient progress in an inpatient rehab setting, serving as a liaison among patients, families, and the healthcare team.

You will complete orientation, identify barriers to discharge, and document care per CMS guidelines.

Qualifications

• Current state RN or Social Work licensure required or healthcare professional licensure as RT/PT/SLP/OT.
• Certification in Case Management or Rehabilitation Nursing preferred (CCM/ARN/ACM/ANCC RN-BC).
• Minimum of 2 years social work or case management experience in an inpatient setting; rehab hospital experience preferred.

Responsibilities

• Coordinating the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting.
• Liaise among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.
• Complete departmental orientation, as well as initial and annual competencies.
• Collaborate with interdisciplinary team members to identify barriers to care or discharge and develop appropriate solutions.
• Document patient care activities in alignment with workflow timelines, including IPoC per CMS guidelines.
• Schedule family conferences and communicate progress and discharge planning with caregivers after team conferences and as needed.
• Coordinate weekly interdisciplinary team conferences to monitor treatment goals and outcomes.
• Review assigned CMG and assist the team in identifying actively treated comorbid conditions; communicate findings to the HIM team.
• Perform other duties as assigned.
