Job summary

Pathway Caring for Children is seeking a Prevention Specialist / Case Manager to deliver prevention education and mental health support in schools and community settings. The role includes CPST/TBS-like case management to address individualized needs and promote stability.

You will collaborate with school staff and families, document thoroughly, and ensure compliance with Ohio guidelines and funders. A valid Ohio driver’s license and willingness to travel are required.

Qualifications

  • High school diploma required; bachelor’s degree preferred.
  • Must meet QBHS credentialing or actively pursue credentialing as Prevention Specialist.
  • Minimum 6 months experience with children, teens, or families.
  • Must have a valid Ohio driver’s license, good driving record, and access to a reliable vehicle.

ROLE OVERVIEWThe Prevention Specialist / Case Manager provides both prevention education and mental health support to youth and families. Prevention services are delivered through evidence-based group programs in schools and community settings. Case management services include CPST/TBS interventions that address individualized needs, support daily functioning, and enhance stability. The position is trauma-informed and community-focused, requiring flexibility and strong collaboration.ESSENTIAL FUNCTIONS AND DUTIESPrevention ResponsibilitiesPlan and facilitate prevention programs (e.g., DINA, Life Skills, All Stars, Strengthening Families).Deliver services in classrooms, groups, or public settings.Collect and report outcomes per program and funder requirements.Tailor presentations and curriculum to match the needs of different age groups or audiences.Case Management ResponsibilitiesComplete needs assessments and develop individualized service plans.Provide CPST and TBS services in homes, schools, and community settings.Support skill development in emotional regulation, daily functioning, and symptom management.Link clients to natural supports, resources, and formal systems of care.Collaboration and Community EngagementWork closely with school staff, therapists, and family members.Participate in community coalitions and planning initiatives.Coordinate services and communicate progress with stakeholders.Documentation and ComplianceMaintain accurate and timely documentation in the electronic health record.Ensure all records meet requirements for medical necessity, OhioMHAS, Medicaid, and funders.Submit reports, logs, evaluations, and outcome data as needed.Professional ResponsibilitiesParticipate in agency and program-specific training.Attend supervision, staff meetings, and ongoing professional development.Adhere to agency policies, ethical standards, and confidentiality practices.Framework AccountabilityIntegrate Trauma Competence into all services, relationships, and planning.Apply Zero Suicide protocols in screenings, safety planning, and crisis intervention.Practice with integrity and uphold Unified Perspectives in all client and team interactions.Model and encourage the agency’s 7 core values – Love, Recognize, Respect, Security, Success, Responsibility and Personal Involvement.Performs duties as assigned by agency leadership.QUALIFICATIONS: The successful applicant must hold a high school diploma, with a bachelor’s degree in a related field preferred. Must meet qualifications for a Qualified Behavioral Health Specialist (QBHS) and be credentialed or actively working toward credentialing as a Prevention Specialist through the Ohio Chemical Dependency Professionals Board. A minimum of 6 months of experience working with children, teens, or families is required. This individual must demonstrate:Consistent application of Trauma Competence and alignment with the Zero Suicide framework.Understanding and modeling of Unified Perspectives, including cultural humility and inclusivity.Skill in delivering both group-based prevention programming and individualized case management.Ability to work across school, community, and home environments.Strong documentation, communication, and time management abilities.A valid Ohio driver’s license, good driving record, automotive liability insurance, and access to a safe vehicle.
