SUMMARY OF DUTIES

Case Manager – Positive Pathways Youth Mentoring Program

The Case Manager for the Positive Pathways Youth Mentoring Program works under the supervision of the Program Supervisor and collaborates closely with Mentors to provide guidance, encouragement, and positive support to youth who are at risk of, or recovering from, substance use, involvement with the juvenile justice system, or other life challenges. Through a strengths-based, trauma-informed approach, the Case Manager helps youth build resilience, strengthen social and emotional skills, and achieve personal, academic, and recovery-related goals.

This role is responsible for conducting outreach, screening, intake, and comprehensive assessment activities while ensuring youth and families are effectively engaged in services. The Case Manager develops individualized, youth and family-driven service plans that reflect participants' strengths, needs, and goals, and coordinates appropriate services and resources to support positive outcomes.

Using evidence-based practices and culturally responsive interventions, the Case Manager provides ongoing case management, advocacy, support, and care coordination for youth and their families. This position works collaboratively with program staff, schools, community partners, and service providers to create a safe, supportive environment that promotes accountability, growth, and self-sufficiency.

The ideal candidate is passionate about serving youth and families, possesses strong assessment and case management skills, and is committed to delivering high-quality, trauma-informed services that empower young people to overcome challenges and reach their full potential.

This position includes community-based activities, school visits, home visits, and office-based services. Flexibility is required, as occasional evening and weekend hours may be necessary to meet the needs of youth and their families.

EDUCATION AND TRAINING REQUIREMENTS

• High school, plus additional courses or specialized training at business or community college, or other formal training facility orprogram. Requires ability to understand and carry out assigned duties effectively. Knowledge needed such as stenography, office routines, elementary accounting procedures; data entry and basic computer skills
• Other (Specify): Bachelor’s Degree preferred. Knowledge of a specialized field or may lead to licensing (human services), or formalized apprenticeship program preferred.

EXPERIENCE

• Over two years, up to and including four years.
• Other (Specify): Two years' experience working with at-risk youth, preferably youth who have been involved with the juvenile justice system

SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED

• Participates in the implementation of a trauma-informed approach to education and behavior management, that promotes relationship building, pro-social behaviors, cooperative learning, self-help skills, academic achievement, and strengthening daily living skills within an academic environment

OTHER POSITION REQUIREMENTS

• This job requires a valid Hawaii driver's license, a clear driver's abstract, and use of personal vehicle to transport participants,as needed, as well as a willingness to travel in personal vehicle on an as-needed basis throughout the island
• This position is subject to a criminal background investigation and continued employment is contingent on a record clear of convictions
• Continued employment in this position is contingent on successful completion of CPR, CPI and/or van driving training as such training is necessary to ensure ability/continued ability to perform essential functions of this job.
