Job Description

The Case Manager is responsible for developing a goal-directed, age-appropriate plan of care through an interdisciplinary team process, based on medical diagnosis, patient needs, and expected outcomes. The role includes collaborating with the team to create and document discharge plans, advocating for patients and families, and ensuring activities to facilitate, broker, and coordinate the plan are implemented and modified as needed.

Responsibilities include:

  • Negotiating services and resources for complex discharge planning.
  • Participating in discharge planning rounds and process improvement projects.
  • Communicating patient needs and updates to relevant professionals and post-acute providers.
  • Ensuring continued appropriate hospitalization through collaboration with the multidisciplinary team.
  • Maintaining thorough and timely documentation of the discharge plan.
  • Completing initial screens within 24 hours or the first business day.
  • Providing education and resources to the team regarding levels of care, transition planning, and regulatory issues.
  • Staying informed on current research, trends, reimbursement modalities, community resources, and legal considerations.
  • Assisting in arranging and following up with community resources for seamless patient transitions.

Qualifications:

  • Bachelor's degree in Nursing (BSN) required; Master's preferred.
  • Graduate of an accredited nursing school.
  • Virginia RN licensure required.
  • BLS certification required; certification in Case Management is desirable.
  • Minimum five years of acute care experience preferred; prior case management experience a plus.
  • Skills in communication, critical thinking, organization, and problem-solving required.
