Position: Case Manager 2

Location: Bridgeport

Salary: $22.00/hour

Schedule: Full time, 40 hours, 1st shift, Monday-Friday

Program Summary:

The Start Program is a nationally recognized program that works with homeless and unstably-housed youth, age 18-24, located throughout the state of CT through an innovative service delivery model that combines short-term rental assistance with intensive, community-based case management services targeted towards housing stabilization. Through a lens of trauma-informed care and positive youth development, the program partners with young adult participants to identify goals and resources that help them to exit homelessness and achieve long-term stability.

Position Summary:

The Case Manager 2 is responsible for providing home- and community-based case management services to homeless young adults located throughout the state. Responsibilities include:

  1. Delivering comprehensive case management services to clients in their homes and in their communities.
  2. Conducting intake assessments.
  3. Assisting youth in locating available apartments for rent.
  4. Working in partnership with youth to develop and implement individual service plans that will progress them to self-sufficiency.
  5. Liaising with treatment providers and community resources.
  6. Providing transportation to clients as needed.
  7. Developing money management skills with caseload.
  8. Developing employment readiness skills with caseload.
  9. Liaising with landlords and property management staff to support youth’s continued tenancy.
  10. Data entry and documentation.
  11. Managing a caseload size that will vary between 8-12 young adults at any given time.

Working within our Young Adult Services, the Case Manager 2 works with the START program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness.

Requirements:
  1. Associate’s Degree
  2. 2+ years relevant experience in direct service and case management field
  3. Valid Connecticut Driver’s License
  4. Regular and predictable attendance is required
Orientation:

If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
