Ernest Health, Inc. at Elkhorn Valley Rehabilitation Hospital in Casper, Wyoming seeks a Case Manager to join a team of patient caregivers on a per diem basis.

You will work with physicians and a multidisciplinary team to develop care plans, monitor progress, and assist with discharge planning, while maintaining high standards of quality and confidentiality. The role emphasizes understanding treatment costs, community resources, and effective communication to support patients and families

Qualifications

  • Current clinical licensure strongly preferred.
  • One year of recent relevant experience; acute care setting preferred.
  • Demonstrates understanding of treatment costs and financial support as they relate to quality and efficiency.
  • Demonstrates critical thinking skills: using logic and reasoning to identify strengths/weaknesses of solutions.
  • Utilizes leadership skills in delegating, organizing, and educating coworkers and staff.
  • Knowledge of community resources to meet post-discharge clinical and social needs.
  • Knowledge of clinical operations and procedures.
  • Ability to establish and maintain proactive relationships with internal interdisciplinary team members, local payer, physicians, community service organizations and health care facilities.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Demonstrates general computer skills including: data entry, word processing, email, and record management.
  • Effective organizational and time management skills.
  • Effective written and verbal communication skills.
  • Ability to maintain proper levels of confidentiality.
  • Ability to work closely and professionally with others.

Responsibilities

  • Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients.
  • Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records.
  • Identifies and resolves barriers that hinder effective patient care.
  • Actively involved in discharge planning process.
  • Integrates the hospital’s mission and “Guiding Principles” into daily practice.

Case Manager - Per Diem - Casper, Wyoming

Elkhorn Valley Rehabilitation Hospital has been ranked in the Top 10% nationally for rehabilitative care for 16 years, has earned The Joint Commission’s Stroke Certification and Certification in Hip Rehabilitation, and ranked in Newsweek’s top Physical Rehabilitation Centers in the US. At our 40-bed hospital, we provide specialized inpatient and outpatient rehabilitative services to our patients, serving Wyoming and neighboring states. We see patients who often are recovering from disabilities caused by injuries, illnesses, or chronic medical conditions.

Qualifications
Required Skills:
