Institutes of Health is seeking a dedicated Case Manager to join our state-of-the-art facility, combining Psychology, PM&R, Neurology, and Psychiatry under one roof. You will interface with clinics, physicians, and patients, obtaining authorizations and coordinating care with our multidisciplinary team.

This role requires a high school diploma, strong writing skills, and bilingual proficiency in English and Spanish.

Qualifications

Responsibilities

• Interfacing with referring clinics, physicians, insurance adjusters and patients.
• Support physicians in patient care within a multidisciplinary team.
• Obtain authorizations for treatment.
• Obtain medical records.
• Verify patient information and current condition.
• Coordinate patient care with scheduler.
• Arrange patient transportation.
• Counsel patients on physician orders.
• Update and verify patient information in EMR.
• Follow up on appointments, authorizations and records.
• Provide support to coworkers.
• Cross-train and back up coworkers.

PRIMARY RESPONSIBILITIES

• Interfacing with referring medical clinics, physicians, insurance adjusters and patients.
• As part of a multidisciplinary team, supports our physicians in the delivery of care to patients.
• Obtaining authorizations for treatment.
• Obtaining medical records.
• Verifies patient information, history taking, confirmation of current condition.
• Coordinate patient care with scheduler.
• Arranging patient transportation
• Counsels patients on physician orders.
• Updates and verifies patient information in our EMR system
• Following up on medical appointments, authorizations and requests for medical records
• Providing support to coworkers.
• To be crosstrained and back up to coworkers

REQUIRED QUALIFICATIONS

• High school graduate.
• Strong writing skills.
• Bilingual in English and Spanish.
