Job summary

Community Reach Center is seeking a part-time Case Manager to support outpatient and community programs in Brighton. You will assist clients in developing social and living skills, linking them to essential resources, and coordinating care with clinical teams.

The role emphasizes prevention, early intervention, and service planning, with a focus on empowering individuals in the community to achieve their goals. Schedule is 20 hours weekly, with some in-home visits.

Qualifications

  • Requires bachelor's degree in Human Services or related field.
  • Strong communication, organization and time management.
  • Ability to work independently in a community-based setting.
  • Valid driver’s license with clean driving record.

Responsibilities

  • Provide case management and family psychoeducation.
  • Coordinate mental health care with clinical team and primary care.
  • Attend care team meetings and reviews of consumers.
  • Provide crisis de-escalation when needed.
  • Assist with access to medical, social, educational and other services.
  • Develop needs assessments and service plans.
  • Support consumer goal attainment and model positive behavior.
  • Provide transportation for consumers as needed.
  • Collaborate with outside organizations for resources and care coordination.
  • Track outcomes using evidence-based measurement.

About the Role:

The Case Manager is an integral member of Community Reach Center’s Outpatient, Community programs, and Case Management Teams. The Care Manager role will emphasize assisting persons to explore and develop social, occupational, leisure, and living skills. Provides services that supports consumers in living in the community successfully including linking consumers with resources to obtain and/or learn basic needs.

Essential Functions:
  • Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families
  • Support and closely coordinate mental health care with the consumer’s clinical care team and the primary care provider
  • Attend collaborative treatment team meetings and reviews of consumers
  • Provides crisis de-escalation when needed
  • Assist eligible consumers in gaining access to needed medical, social, educational, and other services
  • Provide an assessment of needs and service plan development
  • Empowers consumers to reach their goals and sets a positive example and provides role modeling as well as technical expertise to accomplish goals
  • Provide transportation for consumers in agency vehicles when needed
  • Will work closely with outside organizations on resources and care coordination
  • Using an evidence-based model of measurement-based care tracks success for each individual consumer
Qualifications:
  • Requires bachelor's degree Human Services or related field
  • Communication, organization, time management and clinical skills.
  • Ability to work independently in a community-based setting.
  • Valid Colorado Driver’s license with a clean driving record.
  • Bilingual Spanish preferred, but not required
Physical Requirements:
  • Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and keyboard.
  • Specific vision abilities required by this job include close vision requirements due to computer work.
  • Light to moderate lifting is required,
  • Regular, predictable attendance is required.
Schedule:

Schedule: 20 hours per week, Schedule negotiable during outpatient business hours; primarily office based at Brighton Outpatient with some community/in home visits

Salary Information:

$24.04 to $27.00/jhr

Eligible for $250/month Bilingual Stipend
