Job summary

The Colorado Coalition for the Homeless in Denver is seeking a Case Manager I to collaborate with case managers, supervisors, integrated health team members, and community partners to promote long‑term solutions for individuals and families experiencing homelessness. You will develop and implement care plans, coordinate services, monitor client progress, advocate, and maintain documentation in the EHR.

Qualifications

• 1–2 years experience with mental health challenges or homelessness.
• Experience with case management and trauma‑informed care.
• Bilingual or multilingual skills preferred.

Responsibilities

• Manage a caseload of clients with high service needs and disabilities.
• Coordinate care plans and services across health, housing, and social supports.
• Maintain comprehensive case documentation and data collection.
• Provide crisis intervention and de-escalation when needed.
• Advocate for clients with internal teams and external partners.
• Facilitate referrals to external agencies and housing resources.
• Transport clients as needed and accompany residents to appointments.
• Document all services in the EHR accurately and timely.
• Engage in community collaborations and ongoing trainings.
