Job summary

The Children’s Home of Reading is seeking a compassionate Case Manager for the ORR Program to support UACs across two PA locations. You will maintain caseloads, meet weekly with youths, coordinate release planning with sponsors, and document activities in the UAC Portal for timely reporting.

Qualifications

• Bachelor’s degree in behavioral sciences, human services, or social services fields.
• Must be at least 21 years of age.
• Must exhibit integrity and good moral character for working with children in a residential or foster care setting.
• Must complete ORR-required background investigations.
• Must provide proof of immunity to vaccine-preventable diseases and comply with health screening requirements.
• Must complete ORR-required pre-service training and continuing education.
• Must sign CHOR’s Code of Conduct and mandatory reporter agreement.
• Must disclose any prior misconduct on the employment application.

Responsibilities

• Maintains a caseload consistent with ORR case management ratio requirements.
• Provides weekly individual meetings with assigned UACs.
• Coordinates communication with sponsors regarding release planning.
• Manages release planning activities, including home study coordination and travel arrangements.
• Documents all case activity in the UAC Portal in a timely and accurate manner.
• Coordinates with foster families, clinicians, and Lead Case Manager on service needs.
• Provides child-facing services on-site within the assigned service area.
• Participates in case review meetings.
• Completes required training on child welfare, release planning, and mandatory reporting.
• Exercise independent judgement consistent with program policy, ORR requirements, and applicable law.
• Communicate effectively with children, families, and staff.
• Maintain regular, reliable attendance and respond promptly to program needs, including outside standard business hours.
