Altior Healthcare is seeking a dedicated Case Manager to join our Ridge team. You will collaborate with clinical staff to assess, plan, and support clients and families through treatment and discharge.

• Participate in interdisciplinary treatment planning and proactive discharge planning.
• Educate clients and families regarding community resources, benefits, and coverage.
• Coordinate meetings between clients, families, and residential care team; assist in difficult decisions while honoring preferences and values.
• Facilitate group sessions and support recovery-focused programming.
• Document all clinical contacts and documentation through Electronic Medical System.
• Support a safe, structured, trauma-informed residential environment.

As the Ridge Case Manager ,you willcollaboratewith other members of the clinical team,toserveas a liaison for the treatment team, client,familiesandaftercare services.

Key Responsibilities:

• Provide comprehensive case management, including assessment, care planning, and ongoing support.

Qualifications:

Salary Range

$28-$30 USD
