EAC Network is hiring a full-time Case Manager for the Community Services Program in Hempstead, NY. You will conduct client intake, refer clients to suitable volunteer worksites, and monitor progress while maintaining confidential records.

The role requires strong communication, organizational and interpersonal skills, a NY driver’s license, and proficiency with MS Office. Bilingual English/Spanish is a plus; hybrid/onsite options are available.

Qualifications

  • High School Diploma or equivalent required; college degree preferred.
  • Proficient in MS Office, calendars, email, and database software.
  • Excellent communication and interpersonal skills; valid NY driver's license.
  • Bi-lingual English/Spanish a plus.

Responsibilities

  • Conduct client intake interviews and collect client fees.
  • Match clients to appropriate worksite placements and conduct client follow-up.
  • Identify and link clients with supportive services.
  • Maintain accurate client records and have available for inspection by Supervision at any time.
  • Log information in the program database per established timeframe.
  • Monitor clients while they are performing their community service assignment and report any problems, issues or concerns regarding the clients progress.
  • Submit client completion forms and other required for submission to the community justice referral source.
  • Assist in maintaining information systems and tracking of referrals.
  • Provide required client information to ensure prompt entries into database.

EAC Network, a leading not-for-profit social service agency dedicated to helping over 80,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Case Manager to join the Community Services Program.Wage: $20/ hourSchedule: 9AM-5PM, Monday- FridayLocation: 175 Fulton Ave 4th Floor in Hempstead, NY, 11550What drives the core purpose of this job:The Case Manager is responsible for working directly with offenders involved with the Community Service Program.
