Job summary

Qualifications

• Two+ years professional social work or healthcare experience preferred.
• Case Management or related experience preferred.
• Weekend and holiday coverage as needed; MS Office proficiency preferred.

Responsibilities

• Serve as resource for discharge planning and care coordination for psychosocially complex cases.
• Conduct follow-up phone calls.
• Document in Care Conference records.
• Facilitate coordination of care across disciplines.
• Maintain knowledge of community resources and post-acute options.

What We Offer

As a Case Manager MSW at Novant Health Charlotte Orthopedic Hospital you'll support our Inpatient Rehab unit as the leader of the patient assessment and complex discharge planning process. Our case managers collaborate with physicians and other team members to plan and implement safe discharge plans.

Charlotte Orthopedic Hospital is the region's only orthopedic hospital, centrally located just outside of uptown Charlotte. Our inpatient rehab unit serves adult patients for short term, multi-specialty rehabilitative care.

What You'll Do

• Serve as a resource for discharge planning and care coordination for psychosocially complex cases


• Follow up phone calls


• Care Conference documentation


• Responsible for facilitating coordination of care


• Maintain current knowledge of available community resources and post acute care options


• Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health

What We're Looking For

• Education: Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.


• Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred.


• Additional Skills (required): Weekend and holiday coverage as needed


• Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding
