Job summary

Novant Health Huntersville Medical Center is seeking a Part-Time Case Manager BSW to participate in discharge planning and care coordination. The role requires collaboration with physicians and other healthcare team members to develop proactive, safe discharge plans and to support psychosocially complex cases.

This position offers a flexible schedule with 3 eight-hour shifts and one full weekend every six weeks.

Qualifications

• Bachelor's Degree in Social Work from a CSWE-accredited program.
• Two or more years of professional social work experience or healthcare experience preferred.
• Case Management experience preferred.

Responsibilities

• Participate in complex discharge planning as a valued member of the healthcare team.
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans.
• Collaborate with RN Case Managers for guidance with medically complex cases.
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as needed.
• Responsible for facilitating coordination of care.
• Maintain current knowledge of available community resources and post acute care options.
• Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health.

As a Case Manager BSW at Novant Health Huntersville Medical Center you will participate in the complex discharge planning process. Huntersville Medical Center i s an award winning 151 bed hospital in Huntersville, a family-friendly town in the Lake Norman area.

This part-time position offers you more flexibility in your schedule while still offering part-time benefits. The schedule will rotate with 3 8 hour shifts and one full weekend every 6 weeks. You must be able to orient on a Monday - Friday basis for 4-8 weeks depending on experience.

What You'll Do

What We're Looking For

• Education: Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education is required.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred.
• Additional Skills (required): Weekend and holiday coverage. Must work one full weekend every 6 weeks.
• Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
