Oak Plains Academy is a 98-bed residential treatment program delivering psychiatric services to adolescents. The Case Manager (MSW) joins a multidisciplinary team to provide case management, admission support, and discharge planning, supporting quality treatment for residents and families.

With experience in crisis management and family therapy concepts, you’ll work as part of a larger UHS network to promote safety and trauma-informed care in a rural Tennessee setting.

Formación

• Master’s degree in Social Work required.
• Licensed or eligible for LMSW/LCSW/LPC/LMFT in relevant field.
• 1–3 years experience with psychiatric behavioral health population; 3–5 years preferred.
• Background check and driver’s license required.

• Conduct utilization review activities and coordinate with care providers.
• Assist treatment team with discharge planning and age-appropriate growth and development support.
• Apply crisis and behavior management principles; support individual and family therapy concepts.

Oak Plains Academy, (a UHS company) is seeking a dynamic and talented Case Manager (MSW) to join our team of professional employees.

Oak Plains Academy is a comprehensive and highly structured 98-bed residential treatment program offering psychiatric residential treatment services to adolescents, struggling with emotional and behavioral issues. Residents receive an individualized treatment plan, geared to meet their personal needs. A professional team of psychiatrists, psychologists, social workers, therapists, nurses, and teachers serve hundreds of children and adolescents annually with the common goal of treating and healing trauma and building resiliency in the residents we serve. Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy campus features a full-size gymnasium, dining hall, separate academic building, and four dormitories. Our wooded campus provides an excellent opportunity for outdoor recreational activities.

Job Summary:

The Case Manager is responsible for providing quality case management, admission support, utilization review activities and services to residents, their families and care providers. Serve as a member of treatment team supporting the organization’s treatment program and philosophy and assure the deliverance of quality treatment to residents and their families. Knowledge of case management, discharge planning, age-specific growth and development. Crisis and behavior management, individual and family therapy theory practice, behavioral health and substance use.

Qualifications

Requirements:

• Master’s degree in Social Work required
• Licensed, LMSW, LPC, LCSW or LMFT or eligibility for licensure track in social work or related field acceptable.
• Prior experience working with a psychiatric behavioral health population between 1-3 years required, 3-5 years preferred
• This role requires a successful background check, fingerprinting and employment verification
• Valid Driver's License
