Oak Plains Academy in Tennessee is seeking a Case Manager (MSW) to provide quality case management, admission support, and utilization review services to residents and families. You will be part of a treatment team delivering family-inclusive behavioral health care.

The role requires MSW, licensure eligibility, and experience with psychiatric populations; a background check and driver’s license are required. The setting is a structured 98-bed residential program serving adolescents.

Qualifications

• Master’s degree in Social Work is required.
• Licensed, LMSW, LPC, LCSW or LMFT or eligibility for licensure track in social work or related field acceptable.
• Prior experience with psychiatric behavioral health population 1–3 years required, 3–5 years preferred.
• Successful background check, fingerprinting and employment verification.
• Valid Driver's License is required.

Responsibilities

• Provide quality case management, admission support, and utilization review activities for residents, families and care providers.
• Serve as a treatment team member supporting the program’s philosophy and ensure quality treatment delivery.
• Demonstrate knowledge of case management, discharge planning, and age-appropriate development.
• Apply crisis and behavior management skills and knowledge of individual and family therapy concepts.
• Support behavioral health and substance use treatment as part of comprehensive care.

Oak Plains Academy is a comprehensive and highly structured 98-bed residential treatment program offering psychiatric residential treatment services to adolescents, struggling with emotional and behavioral issues. Residents receive an individualized treatment plan, geared to meet their personal needs. A professional team of psychiatrists, psychologists, social workers, therapists, nurses, and teachers serve hundreds of children and adolescents annually with the common goal of treating and healing trauma and building resiliency in the residents we serve. Located on 20 acres of rolling hills in a rural setting, about 45 minutes from Nashville, Tennessee, Oak Plains Academy campus features a full-size gymnasium, dining hall, separate academic building, and four dormitories.
