The Case Manager is a member of the multidisciplinary treatment team. Major functions include conducting psychosocial and CD assessments, gathering information from family and other providers, identifying discharge needs, and completing discharge planning. The Case Manager also assists with placement and other discharge planning needs, including scheduling appointments for aftercare. The Case Manager completes documentation per policy and procedure, including treatment plans and treatment plan updates.

Qualifications

• Licensed in the State of Florida as an LCSW, LMHC, or LMFT.
• Master's Degree from an accredited school in the Behavioral Sciences.
• Two (2) years post‑Master's working experience in group therapy, case coordination activities, substance abuse, mental health, and specific treatment related to adults in acute care and/or intensive outpatient settings.
• Must meet State Law regulations for credentialing clinicians.
• Unlicensed therapists must be Master’s prepared and actively working toward licensure, receiving documented supervision weekly with counter‑signed clinical entries.
