Job summary

Acenda Integrated Health is seeking a Case Manager to provide dedicated case management, outreach, and individualized support to youth in or seeking supportive housing. The role focuses on safety, stability, and access to services addressing mental health, substance use, and other needs.

The position includes coordinating with vocational and educational partners, offering counseling and crisis support, assisting with medication monitoring, and helping youth secure public benefits.

Qualifications

• Bachelor's degree in social service- related field is required.
• 1 year of experience in the field working with children, youth or families.
• Knowledge and/or experience in trauma-informed care, mental health and substance abuse.
• Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record.

Responsibilities

• Provide case management, outreach, engagement, and support to youth residing in or in search of supportive housing units in Burlington County.
• Ensure the safety, stability and well-being of youth and identify services/supports for youth with co-occurring challenges.
• Liaison with vocational and educational community partners to help youth develop career plans and address barriers to employment.
• Provide individual support and counseling and assist with crisis management and medication monitoring.
• Help youth secure public benefits and develop recovery strategies.

Job Overview

The Case Manager provides case management, outreach, and individualized support to youth in or seeking supportive housing, ensuring their safety, stability, and access to services addressing mental health, substance use, domestic violence, economic challenges, and other needs. The role also includes coordinating with vocational and educational partners, offering counseling and crisis support, assisting with medication monitoring, and helping youth secure public benefits and develop recovery strategies.

Key Responsibilities:

• Provide case management, outreach, engagement, and support to youth residing in or in search of supportive housing units in Burlington County.
• Ensures the safety, stability and well-being of youth and effectively work with and identify services/supports for youth who may be facing a range of co-occurring challenges, including mental health, domestic violence, substance use, economic challenges, lack of support, and other needs.
• Liaison with vocational and educational community partners to help youth develop career plans and obstacles to employment.
• Provide individual support and counseling and assist with crisis management and medication monitoring.
• Help youth secure public benefits, and develop recovery strategies.

Requirements:

• Bachelor's degree in social service- related field required.
• 1 year of experience in the field working with children, youth or families.
• Knowledge and/or experience in trauma-informed care, mental health and substance abuse.

Additional Information:

• Base Rate: $19.23
• Sign-on Bonus: $1,000
• Bilingual: An additional $1.50 added to the base rate if Spanish proficient
• Schedule: Two shifts 8:30-4:30p; one shift 11-7p; two shifts 1-9p plus on-call overnights, weekends, and holidays in a rotation
