Job summary

Acenda Integrated Health is hiring a Care Manager Hourly in Egg Harbor Township to support families facing homelessness or housing instability through in-home sessions and evidence-based practice.

You will collaborate with the care team, perform assessments, link clients to community resources, and document progress while maintaining an active caseload of 10–12 families. Day shifts with a $500 sign-on bonus and competitive benefits.

Qualifications

• Bachelor’s level practitioner with at least 2 years in social service related field preferred.
• Must have and maintain a valid driver’s license with an acceptable driving record.
• Ability to think independently and exercise good judgment under all circumstances.
• Ability to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Responsibilities

• Serve as the contact point, advocate and informational resource for clients, care team, family/caregivers and community resources regarding client’s service plan.
• Partner with clients to monitor adherence to their service plan, evaluate effectiveness.
• Monitor progress in a timely manner and facilitate changes as required.
• Facilitate client linkages, referrals and access to appropriate treatment, community supports.
• Maintain an active caseload of approximately 10-12 families, providing in person in-home therapeutic sessions.
• Educate patient and family/caregiver(s) about relevant community resources.
• Cultivate and maintain relationships with external community providers and supports.
• Completes and submits accurate reports and chart documentation in a complete and timely manner.
• Ensures quality and accuracy of written and verbal communications.
• Support the Quality/Compliance/Risk function as required.

Program Overview:

To keep families together by providing safe, permanent housing, and ongoing accessibility to evidence-based trauma informed support services to Child Protection and Permanency families throughout our region.

PositionSummary :

Provide support services and case management to families with a history of homelessness or housing instability.Assistparentsto makea successful transition to self-sufficient living andimprovedindividual and family stability.Provide coaching and guidance on life skills such as home management, nutrition, budgeting, parenting, education/vocation training, advocacyskills.Co-facilitate groups for parents,childrenand families.Link families to both formal and informal community resources.Work collaboratively with your KFT peers, DCP&Pandcommunity service providers.Complete documentation consistent with program and agency requirements.As an integrated healthorganizationAcendavalues the use of evidence-based practice or EBP’s.

Additional Information:

Base Pay: $19.23

Sign-on Bonus: $500

Schedule: Day shift

Bilingual rate: An additional $1.50 added to the base pay if Spanish proficient.
