Spectrum Health Systems, Inc. in Westborough, MA is seeking a Case Manager for inpatient clinical stabilization services. The role is full-time, Tuesday–Saturday, 8:00am–4:30pm, with a pay rate of $21.00–$24.00 per hour and a comprehensive benefits package.

The position involves delivering documented clinical services, developing treatment plans, and coordinating referrals within a multidisciplinary team. A high school diploma is required; licensure/certification is preferred but not mandatory.

Competenze

• Provides documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis problems.
• Develops, implements and revises individual treatment plans for assigned clients with client participation.
• Refers clients to appropriate services in the community as necessary.
• Participates in multidisciplinary team treatment planning, case review and case conferences.
