Job summary

Spectrum Health Systems in Westborough, MA is seeking a Case Manager to provide documented clinical services and case management to clients with mental health, substance abuse, or dual diagnosis. You will develop and revise individual treatment plans with client participation and refer clients to the community as needed.

You will join a multidisciplinary team, participate in treatment planning, case reviews, and conferences. LADC I or CADAC preferred but not required.

Qualifications

• Provide documented individual and group clinical services and case management to clients with mental health, substance abuse, or dual diagnosis.
• Develop and revise individual treatment plans within established timeframes with client participation.
• Refer clients to appropriate community services as necessary.
• Participate in multidisciplinary team treatment planning, case reviews and conferences.
• LADC I or CADAC certification preferred, but not required.
