Job description

Our client is seeking a LCSW/Case Manager/Substance Abuse Counselor in Brooklyn, Bronx, Queens, NY.

CLIENT'S REQUIRED SKILLS & EXPERIENCE

  1. Performs substance abuse assessments, collaborates with case managers to identify mutually agreed upon long and short-term substance abuse related goals, and develops a comprehensive service plan.
  2. Must have a propensity and willingness to learn, persevere, and think outside the box.
  3. Completes HRA 2010E applications.
  4. Completes incident reports and submits to supervisor for review.
  5. Consistent and reliable.
  6. Trains clients in activities of daily living skills.
  7. Serves a caseload of approximately 22 clients.
  8. Home visits and fieldwork are required.
  9. Able to maintain healthy boundaries.
  10. Able to remain calm during moments of tension and stress.
  11. Strong written and verbal communication skills.
  12. Strong time-management, organizational skills, and attention to detail.
  13. Coordinates and supervises efforts to place new HASA clients in vacated units within 30 days from referral.
  14. Coordinates and supervises efforts with case management and maintenance staff to ensure a minimum monthly occupancy rate of 95%.
  15. Provides training to new and current case managers.
  16. Ensures that the home visit policy and procedures are enforced, tracked, and documented.
  17. Performs audits of client charts to ensure comprehensive service planning and accurate charting of client progress.
