Cornerstones of Maine/Rubedo Transitions is seeking a Case Manager and Life Skills/Academic Success Coach in greater Portland, Maine. You will support clients to move through daily life challenges toward healthy, sustainable success, working with our clinical team to teach essential life skills and build confidence.

The role involves a full-time caseload of about 6–7 clients, with a strong emphasis on discharge planning and collaboration to promote personal growth and meaningful lives.

Qualifications

• A college degree is required.
• Experience in coaching or mental health field preferred.

Responsibilities

• Support clients to propel themselves through daily life challenges.
• Manage a healthy caseload of about 6–7 clients.
• Focus on discharge planning for clients nearing program completion.
• Collaborate with the clinical team to teach essential life skills and confidence building.
• Help clients develop productive habits and coping strategies.
