Job summary

California Rehab Institute is seeking a Case Manager (CM) for a full-time on-site role in Century City/Los Angeles, CA. Shifts are 8 hours, Monday to Friday, 8:00am–4:30pm.

The CM coordinates health care decisions, discharge planning, and care transitions to ensure quality outcomes and cost-effective care. Minimum qualifications include licensure as required by state guidelines or a Bachelor's/Master's in health or human services, with hospital-based discharge planning experience preferred.

Responsibilities

Position Summary

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans.

Qualifications

Minimum Qualifications

Previous Experience

• Previous experience in Case Manageent and Discharge Planning preferred.
• CCM Certification Preferred.
