Qualifications

Responsibilities
Participate in multidisciplinary treatment team meetings to develop and update individualized service plans.
De-escalate clients to prevent AMA discharges and support continued treatment.
Serve as family liaison with regular progress updates and involvement.
Maintain organized documentation of client records, legal documents, and property.
Coordinate transitions from detox to residential care and arrange external referrals as needed.
