The Oklahoma Department of Human Services seeks a Case Manager III H21C in Oklahoma City to provide direct and indirect casework services to individuals with intellectual and developmental disabilities and their families.

You will manage caseloads, coordinate resources, act as QIDP, lead the IDT, develop IP, and ensure compliance with federal and state regulations. Travel is extensive; a valid driver’s license is required.

Qualifikationen

• Bachelor’s degree in a human services field is required.
• Three years of professional experience with individuals with intellectual and/or developmental disabilities.
• Possession of a valid Oklahoma nursing license acceptable if applicable.

Aufgaben

• Manages and prioritizes a caseload and provides services to individuals with intellectual and/or developmental disabilities and their families.
• Assists the team in identifying community and agency resources to meet individuals’ needs; ensures necessary services are provided.
• Acts as the individual’s QIDP; advocates and ensures providers meet the individuals’ needs.
• Leads the Interdisciplinary Team (IDT); coordinates IP development, implementation, and modification.
• Coordinates guardianship procedures when needed.
• Provides information to individuals/guardians/family members/advocates for decision-making.
• Solves problems and mediates with providers and team members.
• Participates in staff conferences, meetings, and trainings.
