• Two years' experience in a human services field related to the areas of assignment may be required.

Responsibilities

• Completes a face‑to‑face assessment to determine functional limitations, eligibility for service, progress and need areas at least once every 90 days.
• Develops an Individualized Service Plan (ISP) based on assessment information in collaboration with the consumer and, if appropriate, family or significant others.
• Links to services, resources, and supports specified in the ISP.
• Assist the individual directly with obtaining needed resources and advocating for the consumer.
• Coordinates services with other providers within NCSB and with other agencies and private providers.
• Makes contact with family or significant others to promote ISP implementation and community stability.
• Monitors progress and service delivery, providing monthly service to all individuals on assigned caseload.
