Requirements include a Bachelor's degree in a human services field and considerable mental health experience; a Master's degree is preferred for some assignments.

Qualifications

  • Bachelor's degree in a human services field.
  • Considerable experience in a human services field related to the area of assignment; master's degree preferred in some areas.

Responsibilities

  • Authorize short-term local acute care and crisis stabilization unit (CSU) bed days.
  • Provide discharge planning to consumers in local acute care, CSU and DBHDS State hospitals.
  • Monitor treatment during hospitalization or stay to ensure effective modalities.
  • Serves as an agency liaison for local acute care, CSU and DBHDS State hospital staff and care coordinators.
  • Collaborates with providers to identify and overcome barriers to discharge planning.
  • Coordinate case reviews with the Program Supervisor and other teams as appropriate.
  • Link consumers to community resources and supports in discharge plans.
  • Identify systems issues affecting length of stay.
  • May manage a caseload.
  • Maintain clinical, statistical and financial records per Norfolk CSB standards.
