Job summary

Catholic Charities of Southern Nevada is seeking a Case Manager-II for the Operation HOME! program. You will assess, plan, and coordinate housing-focused services, including rapid housing and supportive housing, working with clients to develop ISP and connect to resources.

Responsibilities include intensive case management, collaboration with partners, and ensuring timely service delivery, while maintaining confidentiality and culturally sensitive practice.

Qualifications

• Associates degree in social work or related field preferred, or equivalent experience.
• Minimum one year of full-time professional casework experience or homeless-services exposure preferred.
• Experience with homeless population; families or children preferred.
• Knowledge of Social Work or Counselor Code of Ethics.
• Must be 25+ years old with valid Nevada driver’s license and auto insurance.
• Bilingual ability highly desirable.
• Flexibility to work after hours and weekends.

Responsibilities

• Interact with provider partners and the homeless community queue.
• Develop Individual Service Plans (ISP) for each client.
• Refer clients to community resources and coordinate referrals.
• Assist with housing-focused case management and move-in support.
• Document case notes and maintain confidential records.
