Job summary

The Nueces Center for Mental Health and Intellectual Disabilities is seeking a Case Manager II to support Coordinated Specialty Care in Corpus Christi, TX. This full-time role provides psychosocial rehabilitation planning, case management, and community-based support to clients aged 15–30 with first episode psychosis (FEP).

You will work under the CSC Team Leader to assess needs, develop individualized plans, coordinate services, and help clients gain independence.

Qualifications

• Bachelor's or advanced degree in a social, behavioral, or human service field.
• Experience in social and rehabilitative services work or counseling.
• Valid Texas driver's license with good driving record.

Responsibilities

• Performs psychosocial rehabilitation planning and instructional work.
• Provides rehabilitation and case management services for assigned caseload.
• Coordinates consumer needs to maintain stability and independence in the community.
• Assists clients in attaining optimum levels of independence and recovery.
