ScionHealth is seeking a Care Coordinator to facilitate patient care across the continuum. You will work with the Interdisciplinary Care Transitions (ICT) team to ensure optimal resource use, service delivery and compliance with review agencies. You will support care planning, implementation and evaluation for diverse patient needs.

The role emphasizes coordination with physicians, payors and post-acute providers, plus ensuring quality and cost-effective care through the transition process.

Qualifications

• RN with state licensure (Required)
• 2+ years in healthcare setting (Required)
• Experience in case management, utilization review or discharge planning (Preferred)
• Must read, write and speak fluent English.

Responsibilities

• Coordinate care with interdisciplinary team and physicians.
• Monitor patient stays for effective care coordination.
• Refer high-risk patients for additional support.
• Act as a patient advocate to support informed decisions.
• Collaborate on rounds and discharge planning with staff.
