Qualifications

• Graduate of an accredited program required for RN. BSN preferred.
• Healthcare professional licensure required as Registered Nurse or LCSW/LSW if required by state regulations.
• Two years of experience in a healthcare setting preferred.

Responsibilities

• Coordinates and facilitates the care of the patient population.
• Monitors all areas of patients’ stay for effective care coordination.
• Participates in interdisciplinary patient care rounds.

Skills
