For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, offering over 160 programs for children, youth, adults, seniors, and those with mental illness.

Job Statement: Under the supervision of the Program Manager, the Case Manager II assesses client needs, arranges services, provides information and referral, and assists in planning and coordinating educational and recreational activities.

  1. Assess needs and provide case assistance to seniors.
  2. Develop and follow through with appropriate care plans.
  3. Maintain records and statistics per agency regulations.
  4. Lead biweekly group discussions.
  5. Coordinate the publication of the center's monthly newsletter.

Qualifications:

  • Bachelor's degree in social work or related field.
  • Proficient in English; bilingual in Spanish, Russian, Chinese, or Korean preferred.
  • Experience working with the elderly and case assistance preferred.
  • Proficient in MS Office, especially Excel.
  • Excellent organizational, time management, and interpersonal skills.
  • Ability to travel within the five boroughs as needed.
