Job Description

For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.

Older Adult Centers promote an active life of seniors. The centers aim to assist them to preserve their independence and health by offering a variety of activities and services such as serving breakfast, lunch, health screenings, social services, educational workshops, class exercises, discussion groups, games, art classes, lectures, workshops, organized outings, and special events.

STATEMENT OF THE JOB:

Under the direction of the Program Manager, the Case Manager II determines the client needs for assistance, arranges for services, provides information and referral, and assists the Program Manager in planning and coordinating education/recreational activities for the Program.

  1. Assess needs and provide information, referral, and case assistance to seniors.
  2. Develop appropriate care plans and follow through to disposition.
  3. Maintain records, case files, and statistics regarding client files and activities according to DFTA and agency regulations.
  4. Lead a group discussion at least biweekly.
  5. Coordinate the publication of the center’s monthly newsletter.

QUALIFICATIONS:

  1. Bachelor's degree in social work or related field.
  2. Good verbal and written command of the English language.
  3. Experience working with the elderly preferred.
  4. Experience with case assistance and entitlement counseling.
  5. Proficient in computer software applications, Excel, and Microsoft Office.
  6. Good time management, organizational, and interpersonal skills.
  7. Able to travel to multiple locations within the five boroughs as needed.
