The role requires a BSN with 2+ years in healthcare or an MSW/MPC with relevant experience, and appropriate licensure. This is a salaried, on-site position in a care-driven environment.

Expected pay for this position is $39.13 - $46.03/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.
The Case Manager (CM) II works in collaboration with the patient/family, physicians and interdisciplinary team members to ensure patient progression through the continuum of care and to independently develop a plan of care for each assigned patient from admission through discharge. Identifies, initiates and manages optimal patient flow/throughput to enhance continuity of care, smooth and safe transitions, patient satisfaction, patient safety, and length of stay management. Utilizes their knowledge and expertise to provide clinical oversight and direction for Case Manager I Mission Partners and to evaluate the individual patient's needs via comprehensive assessment and managing care coordination and discharge planning. Assesses and responds to patient/family needs by coordinating efforts ofthe interdisciplinaryteam members and identifies and resolves barriers that hinder effective patient progression of care.
