Job summary

Volunteers of America Mid-States in Jefferson, KY seeks a Case Manager II to support individuals and families experiencing homelessness through Permanent Supportive Housing and Shelter Plus Care. Qualifications

  • Bachelor’s degree in social work or related field; or at least five (5) years of experience in the field (no degree would qualify).
  • Ability to access local resources and coordinate with service providers; knowledge of community resources and advocacy.
  • Excellent verbal and written communication skills, high degree of organization, creative problem-solving abilities.

Responsibilities

  • Provide outreach to community partners and recruit landlords to work with client families.
  • Perform assessments, develop/monitor case plans, and conduct follow-up activities.
  • Establish linkages with agencies and service providers in the area/community.
  • Provide referrals and coordinate with partners to assist participants in accessing health care and daily living services.
  • Allocate resources to participants according to need and grant guidelines.
  • Educate participants on available services and participant rights.
  • Assist participants in obtaining public benefits and essential documentation.
  • Complete documentation within 48 hours of contact and submit HMIS/EHR data as required.
  • Demonstrate good clinical judgment and culturally appropriate engagement with participants.

PROGRAM: Family Stabilization Program- Shelter Plus Care/ Permanent Supported Housing

JOB SUMMARY:

Case Manager will assist with services to individuals and families experiencing homelessness who have been approved by Office of Social Services to receive Permanent Support Housing/ Shelter Plus Care Services. In tandem with other members of the VOA Home team, the Case Manager will assist clients in accessing medical and mental health services, emergency and long-term housing, substance abuse services, and social services. Case Managers work within a harm reduction and person-centered model.

QUALIFICATIONS:

This position requires a Bachelor’s degree in social work or a related field or at least five (5) years of experience in the field and no degree would qualify for this position.

Skills:

Requires a demonstrated ability to provide a reasonable combination of the following knowledge, abilities, and skills: family needs assessment, knowledge, and ability to access local resources, advocacy, counseling, and working cooperatively with local service providers. Must have a commitment to the principles of the project. Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.

Other Requirements

Must have a personal automobile, valid driver’s license, liability insurance, and be willing and able to travel within the full-service area. Must complete required case management and (Critical Time Intervention) CTI training within 90 days of hire.

RESPONSIBILITIES
  • Provides outreach to community partners and represents VOA values ininteractions with community partners to increase awareness of andrecruit landlords to work with our client families in the program
  • Perform assessment and developing/monitoring case plans andconducting necessary follow-up activities.
  • Establish linkages with appropriate agencies and service providers in the area/community.
  • Provide referrals and coordinating with community partners and other service providers to assist participants in accessing health care services and daily living services.
  • Decide how resources are allocated to participants on the basis of need and eligibility, according to grant guidelines
  • Educate participants on issues, such as supportive services availability and participant rights.
  • Provide assistance to participants in obtaining public benefits.
  • Provide assistance to participants in obtaining a birth certificate, driver’s license, or/and TARC tickets.
  • Provide/refer for supportive services to participants, including personal financial planning services, transportation services, income support services, recovery programs, legal services, and housing counseling for the purpose of overcoming obstacles preventing obtaining stable housing and or emergency shelter.
  • Complete required documentation (including progress notes) within 48 hours of contact
  • Complete and submit the required documentation for entry into the Homeless Management Information System (HMIS) and Electronic Health Record system within 48 hours of entry and service to Program Support Coordinator and Portal billing logs to the Program Manager weekly
  • Demonstrate good clinical judgment in decision making regarding participants.
  • Demonstrate ability to relate to participants and their families in a culturally appropriate manner.
  • Performance Quality Improvement (PQI) duties as assigned by supervision & PQI Committee.
  • Facilitate the Critical Time Intervention (CTI) evidence-based model to program participants.
  • Advocate for the participant, integrating cultural values into their case plan.
  • Identify systemic barriers and communicates with organizational leadership about these barriers to work collaboratively to find viable solutions.
  • Comply with all policies and procedures of the program and the Council on Accreditation, Office of Resilience and Community Services, and VOA Mid Policies and Procedures.
