Qualifications

  • High school completion or equivalent.
  • Three or more years of experience supporting individuals with developmental and intellectual disabilities and autism.
  • Valid driver's license with reliable, insured vehicle.
  • Knowledge of Behavioral Support Plans, Communication Plans, and Individual Support Agreements.
  • Understanding of Adult and Child Protective Services reporting and confidentiality.
  • Strong communication, crisis de-escalation, organizational and computer skills.

Responsibilities

  • Develop Behavior Support Plans and Communication Plans based on assessment of individual client needs and goals.
  • Conduct periodic home visits according to program and client plan requirements to monitor client well-being and service delivery.
  • Respond to and manage crisis situations and utilize Crisis Intervention Teams as needed and appropriate.
  • Provide direct care to assigned clients through community support and in-home support to maintain continuity of care.
  • Supervise, train, and participate in hiring direct support staff while monitoring activities related to client outcomes.
  • Maintain strict confidentiality of all sensitive and protected client information and complete documentation in the electronic records system.

Service CoordinatorThe Service Coordinator has a central role in addressing the comprehensive service needs of individuals with developmental and intellectual disabilities and autism through service coordination and advocacy in home and community settings.This position serves as the primary point of contact for clients, guardians, and Shared Living Providers, while supervising direct service staff, providing direct care to maintain continuity of care, and handling administrative and support tasks to ensure program sustainability.Summary of DutiesDevelop Behavior Support Plans and Communication Plans based on assessment of individual client needs and goals.Conduct periodic home visits according to program and client plan requirements to monitor client well-being and service delivery.Respond to and manage crisis situations and utilize support from Crisis Intervention Teams as needed and appropriate.Provide direct care to assigned clients through community support and in-home support to maintain continuity of care.Supervise, train, and participate in hiring direct support staff while monitoring activities related to client outcomes.Maintain strict confidentiality of all sensitive and protected client information in accordance with agency policies and legal requirements.Complete documentation timely and accurately in the electronic records system to capture services delivered to each client.Compensation and BenefitsThe starting pay range for this position is 25.24 to 26.53 per hour.WCMHS provides a comprehensive benefits package including health, dental, vision, and retirement contribution matching. Our industry-leading paid time off package includes 12.5 flexible holidays and 12 days each of sick vacation days accumulated in year one of employment.Summary of Qualifications and SkillsHigh school completion or equivalent.Team leadership experience.Three or more years of experience providing services to individuals with developmental and intellectual disabilities and autism.Valid driver's license, acceptable driving record, and reliable, insured vehicle access.Knowledge of Behavioral Support Plans, Communication Plans, Individual Support Agreements, and evidence-based practices like Trauma Informed Care.Understanding of Adult Protective Services and Child Protective Services reporting requirements, along with maintaining strict confidentiality.Strong communication, crisis de-escalation, organizational, and computer skills.Workplace ConditionsThe work is conducted across a variety of indoor and outdoor environments, including office spaces, program areas, the community, and client homes, requiring regular driving and travel.
