Qualifications

• Bachelor's degree in a social service-related field.
• One year of experience in a social services-related field.
• Clean driving record and ability to pass a background check and drug test.
• Current Auto Insurance.
• Experience in case management, preferably in substance abuse or mental health.
• Strong communication and interpersonal skills.
• Strong organizational skills and ability to prioritize tasks.

Responsibilities

• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans with the treatment team.
• Provide ongoing case management to ensure timely support during residential program stay.
• Assist clients in accessing services such as medical care, counseling, vocational support, and community resources.
• Conduct progress meetings to review plans, identify barriers, and set goals.
• Collaborate with counselors, therapists and medical staff for coordinated care.
• Maintain accurate client records and documentation per standards.
• Ensure confidentiality and ethical guidelines; provide crisis intervention as needed.

CASE MANAGER - FULL TIME - INTELLECTUAL AND DEVELOPMENTAL DISABILITIES (IDD)

Location: McDonough, GA (travel required through 7 counties)

Job Type: Full-Time

Schedule: Mon-Fri 8am-5pm

Pay: $17.00 per hour

• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans in collaboration with the treatment team.
• Provide ongoing case management services to clients, ensuring they receive timely and appropriate support throughout their residential program stay.
• Assist clients in accessing and engaging with various services, such as medical care, counseling, vocational support, and community resources, to support their recovery journey.
• Conduct regular progress meetings with clients to review treatment plans, identify barriers, and set achievable personal growth and recovery goals.
• Collaborate with other professionals, including counselors, therapists, and medical staff, to ensure coordinated care for clients.
• Maintain accurate and up-to-date client records, documenting interactions, progress notes, and treatment interventions in compliance with organizational and regulatory standards.
• Ensure adherence to ethical guidelines, confidentiality regulations, and policies related to client care and case management.
• Provide crisis intervention and support as needed to ensure clients' safety and well-being during challenging times.

Qualifications

• Bachelor's degree in a social service-related field from an accredited college or university.
• One year of experience in a social services-related field.
• Clean driving record and the ability to pass a background check and drug test.
• Current Auto Insurance
• Demonstrated experience in case management, preferably in a substance abuse or mental health setting.
• Excellent communication and interpersonal skills to effectively engage and build rapport with clients and the treatment team.
• Strong organizational skills, with the ability to manage multiple tasks and prioritize responsibilities effectively.
