FHCSD is seeking a Case Manager-Prenatal to support our prenatal patients by coordinating their care plans. Responsibilities include client enrollment, orientation, patient assessment, appointment follow-ups, and documentation. The role emphasizes excellent customer service, community health, and the importance of quality prenatal care for mothers and infants.

Responsibilities:
  1. Assist in patient flow, ensure test results and referrals are properly recorded, and review medical records for completeness.
  2. Complete documentation accurately and timely, including scanning reports into electronic health records.
  3. Coordinate individual care plans, assist with practical arrangements such as translation needs, referrals, and utilization of services.
  4. Ensure case closure following protocols.
  5. Maintain prenatal logs and provide timely follow-up.
  6. Perform other duties as assigned.
  7. Provide client orientation, education, assessments, and work with the team to meet productivity goals.
  8. Provide pregnancy testing and counseling, offering appropriate referrals.
Requirements:
  1. 1 year experience in perinatal or women's health care preferred.
  2. Bachelor's degree in Health Science or Social Science preferred.
  3. High School diploma or GED required.
  4. Medical Assistant diploma or LVN desirable.
  5. Strong data entry skills.
  6. Ability to work effectively with diverse populations.
  7. Ability to work independently and in a team.
  8. Bilingual in Spanish/English required.
  9. Culturally sensitive with good communication skills.
  10. Knowledge of perinatal programs and care management.
  11. Proficient computer skills for electronic records and reports.
  12. Excellent interpersonal and customer service skills.
