People Incorporated Mental Health Services in Eagan, MN is seeking a Case Manager I or II for the Metro County Targeted Case Management team. You will develop individualized plans, assess needs, and connect clients with mental health services while meeting monthly in the community and maintaining records.

The role offers hybrid work, full-time hours, and a competitive hourly pay range from $20.75 to $25.75 with a $1,000 hiring bonus for external candidates.

Qualifications

• Bachelor’s Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing).
• If no Bachelors Degree in a behavioral science or related field, must meet one of: 3–4 years as a case manager aide; or RN with 3+ years related experience; or equivalent training in psychiatry and community discharge planning.

People Incorporated is seeking a Case Manager I or II to join our Targeted Case Management (Metro County) team to work with clients who are diagnosed with chronic mental illness. We support people in connecting with resources to live as independently as possible. We help them develop self-awareness to make positive plans for the future through frequent meetings in the community. Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.

Hours: Full-Time, Monday-Friday, business hours

Location: Metro County (Twin Cities), Minnesota. Hybrid

Hiring Range:

• Case Manager I - $20.75 - $22.00 Hourly, based on qualifications
• Case Manager II - $24.25 - $25.75 Hourly, based on qualifications
• Hiring Bonus: $1,000 to external candidates

Main Job Duties

• Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
• Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
• Assessing client needs and ensuring continuity of care.
• Making ongoing referrals to resources based on assessment and current client need.
• Maintaining necessary daily records for continuity of service and reports to funding agencies.
• Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
• Attending client monthly meetings to monitor services.

Qualifications: Requirements for All Targeted Case Manager I positions

• Valid driver’s license, excellent driving record and current auto insurance

Education and Experience

• Bachelors Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing from an accredited college or university)
• If no Bachelors Degree in a behavioral science or related field, the employee must meet one of the following:
• Have 3 or 4 years experience as a case manager aide; OR
• Be an RN without a Bachelors Degree and have 3 years or more combined specialized training in psychiatry and work experience consisting of community interaction and involvement or community discharge planning in a mental health setting.

Requirements for All Case Manager II positions

• Valid driver’s license, excellent driving record and current auto insurance

Education and Experience Required for Case Manager II positions

• Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND
• One year of experience in the education or treatment of persons with developmental disabilities or related conditions
• OR Master’s Degree in behavioral science or related field.

Preferred Qualifications for Targeted Case Manager II position

• Experience providing direct services to adults with serious and persistent mental illness
• Experience with case management and commitment proceedings
• Knowledge of Recovery, Motivational Interviewing and Person-Centered approach
