Job summary

OSF HealthCare in Bloomington, IL is seeking a Case Manager I to coordinate patient care from admission through discharge. You will work with the patient, family, physicians, and a multidisciplinary team to develop and manage plans of care and to optimize patient flow and safety.

The role requires RN with 1 year healthcare experience or a BS in social work with licensure, strong communication, and problem‑solving skills.

Qualifications

• RN with 1 year healthcare setting experience (Bachelor's degree of Science in nursing within three years of hire).
• Bachelor’s degree in social work or social services with licensure and 1 year of healthcare setting, discharge planning, care coordination, or OSF Case Management Internship.
• RN license (RN).
• Social Worker License (LSW), Licensed Bachelor Social Worker (LBSW) or Licensed Professional Counselor (LPC) in IL or MI.

Responsibilities

• Develop and manage each patient’s plan of care from admission through discharge.
• Coordinate care coordination and discharge planning with an interdisciplinary team.
• Identify and resolve barriers to patient progression of care.
• Ensure communication is accurate, timely, and aligned with the plan of care.
• Support discharge planning and length of stay management.
