Job summary

Old Pueblo Community Services in Tucson is seeking a Case Manager I to engage and coordinate care for individuals experiencing homelessness and related conditions. You will conduct assessments, develop recovery plans, and support clients toward identified goals within a compassionate, team-based environment.

By performing intake, maintaining HIPAA-compliant records, and collaborating with partner agencies, you will help link clients to essential services while applying evidence-based practices

Qualifications

  • High School Diploma or GED required.
  • CPR/First Aid certification or ability to obtain it.
  • Basic computer skills (Word, Excel, email).
  • Willingness to learn evidence-based practices (De-escalation, Motivational Interviewing, Housing First, Harm Reduction).
  • Self-starter with teamwork, reliability and flexibility.
  • Valid Arizona driver's license and reliable transportation.

Responsibilities

  • Complete the intake process with new clients.
  • Deliver services aligned with the client's recovery plan to meet goals.
  • Update recovery plan as services are completed.
  • Assess risk using C-SSRS and coordinate with higher-level staff as needed.
  • Connect clients to mainstream services and other organizations.
  • Provide case management and life skills sessions.
  • Co-facilitate life skills groups with a senior Case Manager.
  • Facilitate ART meetings as needed.
  • Document services in the EHR and maintain HIPAA-compliant files.
  • Monitor client compliance with program guidelines.

The Case Manager I is responsible for the engagement, assessment, support, and coordination of care for individuals who have experienced homelessness. The role involves interacting with individuals who are experiencing homelessness, Serious Mental Illness, Substance Use Disorder, and other related conditions.

Duties and Responsibilities:

  • Complete the intake process with new clients.
  • Deliver services in alignment with the client's recovery plan to assist and support clients in reaching identified goals.
  • Update the recovery plan in collaboration with a Case Manager II as services are completed.
  • Complete the Colombia Suicide Severity Rating Scale (C-SSRS) and coordinates with a Case Manager II or above to address moderate and high need risk plans.
  • Connect clients to mainstream services and other service organizations as needed.
  • Provide case management and life skills individual sessions.
  • Co-facilitate life skills groups with a Case Manager II or above.
  • Facilitate Adult Recovery Team (ART) meetings when needed with guidance from a Case Manager II or above.
  • Document services and information within the Electronic Health Record (EHR).
  • Maintain all client files in a confidential manner and in compliance with HIPAA standards.
  • Monitor clients' compliance with program guidelines and provide appropriate guidance to those clients who are out of compliance.
  • Work closely with OPCS team, collaborators and other members of a client's support team, including Home Health Care agencies, parole and probation offices.
  • Other duties as assigned.
Requirements
  • High School Diploma or GED.
  • Possess and maintain valid CPR/First Aid Certification, or ability to complete training and pass certification.
  • Basic computer skills (e.g., Microsoft Word, Excel, email)
  • Willingness to learn and practice evidence-based practices, such as De-escalation (CPI Model), Motivational Interviewing, Housing First and Harm Reduction.
  • Self-starter committed to a team approach, responsible, dependable, ability to set priorities, meet deadlines and work flexible hours.
  • Valid Arizona driver's license, proof of auto insurance and reliable transportation.
Preferred Qualifications:
  • One year of experience working with the unsheltered population.
  • Experience using an Electronic Health Record (EHR).
  • Bilingual.

Pay Rate - $18.88 - $23.10 - $27.32

In accordance with pay transparency, the salary reflected in this posting is the full salary range for this position. Individual placement within the range is based on the candidate's current experience, education, skills, and abilities related to the position. Salary placement is typically between the minimum and mid-point of the salary range.
