Adelphoi is seeking a compassionate Foster Care Case Manager to join our Huntingdon, PA program. You will support youth and families through home visits, develop individualized service plans, and respond to crises to promote resilience and positive outcomes.

The role involves maintaining EHR records, participating in recruitment events, and conducting family interviews and home studies, with a focus on collaboration and growth within a supportive team.

Qualifications

• Bachelor’s degree in Human Services or 2 years of extensive experience in Child Welfare.
• Valid driver’s license and car insurance.
• FBI, Act 33, and 34 clearances required (agency assistance provided).

Responsibilities

• Provide ongoing support to youth and foster families through regular home visits.
• Develop individualized service plans to support emotional, intellectual, and social growth.
• Respond to crisis situations and provide immediate assistance when needed.
• Maintain accurate records in our Electronic Health Record system.
• Plan and attend recruitment events, presenting to community groups and organizations.
• Conduct family interviews and complete home studies for foster and kinship families.
