Job summary

SSM Health Rehabilitation Hospital in Richmond Heights, MO, is hiring a Case Manager for a full-time role Monday through Friday. You will coordinate care decisions, discharge planning, and ensure cost-effective delivery across the continuum.

Qualified candidates have licensure in a clinical discipline (RN preferred) or a relevant Bachelor's/Master's degree; CCM certification is a plus and rehab experience is preferred. We offer comprehensive benefits and a supportive work environment.

Position: Case Manager

Location: Richmond Heights, MO

Schedule: Full-Time Monday through Friday 8 hour days

Compensation: $77,000-$105,000 based on years of experience

Responsibilities

The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Qualifications

Minimum Qualifications
